A paradigm change in sudden cardiac death risk prediction: ‘static’ goes out, ‘dynamic’ comes in

L’editoriale si riferisce a un articolo apparso sulla stessa rivista[1]Pham HN, Holmstrom L, Chugh H, et al. Dynamic electrocardiogram changes are a novel risk marker for sudden cardiac death. Eur Heart J 2024;45:809–19. https://doi.org/10.1093/eurheartj/ehad770 nel quale il gruppo del Cedars-Sinai Medical Center, Los Angeles, guidato da Harpriya Chugh, propone una valutazione “dinamica” di uno score predittivo di morte cardiaca improvvisa (Electrical Risk Score – ERS -)[2]Aro AL, Reinier K, Rusinaru C, et al. Electrical risk score beyond the left ventricular ejection fraction: prediction of sudden cardiac death in the Oregon Sudden Unexpected Death Study and the … Continua a leggere basata sull’analisi di almeno due elettrocardiogrammi ottenuti nello stesso paziente a distanza di tempo (mediamente in quella casistica 4-6 anni). In particolare le variazioni riscontrate in due parametri, la durata del QT corretto (nella Figura QTc) e del tempo picco/fine dell’onda T (nella Figura TpTe), forniscono informazioni prognostiche analoghe a quelle offerte dall’ERS. Gli autori, dell’editoriale, osservano come l’importanza delle anomalie della ripolarizzazione ventricolare sia da tempo riconosciuta come cruciale nell’aritmogenesi della morte improvvisa e come lo studio presente vada in quella stessa direzione. Altri parametri, quali l’onda T alternante (TWA nella Figura) e l’eterogeneità delle onde T (TWH nella Figura), sono state proposte quali misure quantificabili della dispersione della ripolarizzazione ventricolare. Anche l’analisi spettrale in bassa frequenza, denominate “Periodic Repolarization Dynamics” (PRD nella Figura) permette l’’identificazione di pazienti elettricamente vulnerabili, soprattutto in relazione ad attivazione simpatica secondaria a ischemia miocardica. Tuttavia, queste valutazioni sono “statiche”, eseguite su un solo elettrocardiogramma, mentre la novità dello studio del gruppo di Los Angeles consiste in una valutazione temporale dinamica. Il primo approccio di questo tipo risale addirittura al 1978, quando Schwartz e Wolf[3]Schwartz PJ, Wolf S. QT interval prolongation as predictor of sudden death in patients with myocardial infarction. Circulation 1978;57:1074–7. https://doi.org/10.1161/01.CIR. 57.6.1074. pubblicarono un’esperienza relativa a 55 pazienti con infarto miocardico e 55 controlli sani “matchati” per età, sesso, occupazione, etnia, peso e altezza. Veniva registrato un ECG ogni 2 mesi per 7 anni: 28 pazienti del primo gruppo ebbero una morte improvvisa: nel 36% di essi vi era un QT>450 msec (contro l’8% dei controlli) ma, soprattutto, fu osservata una significativa maggiore variabilità di QT tra i pazienti deceduti rispetto ai sopravvissuti (vedi Tabella da ref. 3).
