La doppia terapia antiaggregante (DAPT) per 12 mesi è un caposaldo del trattamento delle sindromi coronariche acute (ACS) sin dalla pubblicazione dello studio CURE nel 2001. L’evoluzione delle conoscenze ha portato dapprima all’utilizzo di inibitori del recettore piastrinico P2Y12 (P2Y12-i) più potenti di clopidogrel e successivamente a una riconsiderazione della durata della DAPT sulla base di un ridimensionamento del rischio della trombosi dello stent (grazie all’utilizzo di materiali meno trombogenici e alla riduzione degli eventi ischemici per una più efficace prevenzione secondaria) e di una maggiore consapevolezza delle conseguenze connesse al rischio emorragico…

There were fewer major bleeding events with asundexian than with apixaban during this time.

CHEST PAIN IN A MIDDLE-AGED MAN

Un paziente sulla cinquantina si presenta al PS per un angor da circa 30 minuti. In anamnesi un infarto non-ST elevation due anni prima. La PA è 100/60 mmHg, la frequenza cardiaca 75 bpm. Il primo valore di troponina risulta negativo. Qual è l’interpretazione dell’elettrocardiogramma? Trattasi di un tracciato ischemico con presenza del segno di De Winter(1) in un paziente con sindrome di pre-eccitazione ventricolare di tipo A. La pre-eccitazione è rivelata da onde delta. Il sottoslivellamento di ST può inizialmente far pensare a una sindrome coronarica acuta non-ST elevation (NSTE-ACS), per il sottoslivellamento di ST presente nelle derivazioni precordiali, ma il tratto ST è concavo, non discendente ed è seguito da onde T positive e appuntite, chiaramente indicando il pattern segnalato da De Winter e coll.[1]de Winter RJ, Verouden NJ,Wellens HJ, Wilde AA; Interventional Cardiology Group of the Academic Medical Center. N Engl J Med. 2008;359:2071-2073. … Continua a leggere, rappresentativo di occlusione acuta dell’arteria discendente anteriore (osservato nell’1-2% dei casi). Il riconoscimento di questo segno elettrocardiografico è molto importante, perchè è equivalente a un sopraslivellamento di ST e perciò indica la necessità di una coronarografia urgente e di una rivascolarizzazione immediata. Il suo mancato riconoscimento può ritardare l’accertamento angiografico ed esporre il paziente a un rischio elevato. La coronarografia ha in effetti mostrato un’occlusione prossimale dell’arteria discendente anteriore, trattata con successo con una PCI primaria con stenting del vaso riperfuso. Interessante la sequenza elettrocardiografica successiva che ha mostrato la scomparsa dei segni De Winter e assenza di onde Q nelle derivazioni precordiali. Tuttavia, il terzo battito non mostra la pre-eccitazione (vedi Figura 2, freccia). In questa sequenza sono individuabili onde Q in V1 e V2 con ST sopraslivellato fino a V5 (non registrata la V6), assenti nelle sequenze dove la pre-eccitazione è presente, in quanto l’onda delta positiva maschera la presenza dell’onda di necrosi. Il paziente ha avuto una buona evoluzione clinica con un picco di troponina di 4.570 pg/mL (reference 28 pg/mL). CHEST TIGHTNESS WITH QR AND ST-SEGMENT ELEVATION IN LEAD V1.

Un paziente sulla cinquantina lamenta dispnea da circa due mesi dopo una infezione da Covid. All’ingresso la frequenza cardiaca è 118 bpm, la pressione arteriosa 105/70 mm Hg. Tra i dati di laboratorio spicca un valore di NTpro-B di 2.377 pg/mL; vi è anche un lieve aumento di troponina e valori normali di D-dimero.

CHEST PAIN WITH EXERCISE-INDUCED LEFT BUNDLE-BRANCH BLOCK. IS THERE A CONNECTION?

Un paziente sulla sessantina lamenta da circa due settimane dolore toracico durante attività fisica. Il test ergometrico riproduce il sintomo e viene interrotto al secondo stadio del protocollo di Bruce. Il blocco di branca sinistra (BBS) compare durante il test a una frequenza cardiaca di 107 bpm in coincidenza con l’angor e nel recupero scompare, contemporaneamente alla cessazione del dolore toracico riducendosi la frequenza (vedi Figura 1, pannelli A e B). Un caso simile è stato presentato in un precedente numero di Omaggio all’elettrocardiografia di Journal Map. Qui, tuttavia, il contesto clinico è differente.

MAKING THE RIGHT DIAGNOSIS: SLOWLY BUT SURELY.

Un paziente di 84 anni con una severa cardiomiopatia e un PM-defibrillatore doppia camera, impiantato per aritmie ventricolari, si presenta in ambulatorio per malessere da circa 4 giorni. Non ha avuto episodi sincopali nè cardiopalmo. All’esame obbiettivo è in compenso emodinamico, orientato, apirettico, la PA è 110/70 mmHg. Viene registrato l’ECG riportato in Figura 1. La frequenza cardiaca è 96 bpm.

DANGER OF AN OVER-THE-COUNTER PANACEA.

Un signore di 28 anni giunge in Pronto Soccorso per palpitazioni e sensazione di parestesie alla rima orale. Non riferisce patologie nella sua storia clinica. Per lombalgia ha assunto per tre giorni un preparato hindi a base di erbe, chiamato Ramban Ras. Non vi sono dati rilevanti all’esame obiettivo se si esclude un ritmo irregolare, la PA è 110/70 mmHg. L’ecocardiogramma è normale. Viene eseguito un elettrocardiogramma.

DON’T JUDGE A BOOK BY ITS COVER.

Una donna di 55 anni, ipertesa, diabetica, affetta da cardiomiopatia ipertrofica, portatrice di defibrillatore impiantabile in prevenzione primaria, si presenta al Pronto Soccorso per episodi di cardiopalmo associato a dispnea. Riferisce che il defibrillatore è intervenuto molte volte nelle ultime ore; ha eseguito un ECG Holter che ha registrato il tracciato evidenziato in Figura 1 e che è stato interpretato come tachicardia ventricolare.

ELECTROCARDIOGRAM INTRICACIES IN A PATIENT WITH PROSTATE CANCER. THE HEART OF THE MATTER.

Un paziente sulla sessantina si presenta al PS per costrizione toracica e dispnea 1 settimana dopo la seconda dose di toripalimab, un inibitore del checkpoint immunitario, assunto al dosaggio bisettimanale di 240 mg per un cancro prostatico metastatico. All’ingresso la pressione è 145/82 mm Hg, la frequenza cardiaca 105 bpm con una saturazione di O2 di 95% in aria ambiente. L’ecocardiogramma mostra una moderata disfunzione sistolica del ventricolo sinistro (FE 44%). Risulta alto il valore di troponina I (1.376 pg/mL, reference, <19.8 pg/mL), nella norma invece il valore di NTproBNP (87 pg/mL, reference, <125 pg/mL) e la potassiemia (4.81 mEq/L). Un ECG eseguito un mese prima era nella norma, mentre quello registrato all’ingresso è mostrato nella Figura.
